Emily Kornhoff is the Administrator and began working at Beaver Dam Health Care Center in January of 2021. She previously worked at Mayo Clinic as an Exercise Physiologist for 6 years. Emily has a bachelor’s degree in Kinesology from University of Wisconsin – Eau Claire and a master’s degree in Healthcare Administration through the University of Minnesota. She has had a passion for working with the senior population for as long as she can remember. Emily loves what she does and thoroughly enjoys knowing that what she does makes a positive impact on her residents quality of life each and every day. She enjoys spending her free time with her family and friends and doing anything that is active.

Emily Streekstra leads the Beaver Dam Health Care Center nursing department as the Director of Nursing. Emily has served here for the last 3 years, giving of her health care experience of 23 years. Not only does Emily serve as the Director of Nursing but also as the Infection Control Officer. Emily takes time to educate staff on our policies and procedures, implement new and changing precautions. Emily works diligently to serve our staff and residents. Emily is an irreplaceable team leader, who consistently gives of herself.

Brittany Falk is the Assistant Director of Nursing as well as our Wound Care Nurse. She has been a nurse since 2013, earning her BSN in 2019 from Chamberlain University. Brittany grew up in Beaver Dam, a graduate of Beaver Dam High School and has returned to the area to raise her own children. Brittany has a diverse nursing background including ICU and hospice but has a true passion for geriatrics, adding to our varied experiences here at BDHCC.
Monica Saucedo gives to our Beaver Dam Health Care Center as the Director of Admissions. Monica has 9 years of long-term care experience and is currently working towards her LPN diploma. Monica adds to her contributions by being a Spanish speaking staff member, aiding in the transition for multiple residents and short-term rehab clients. Monica enjoys spending time in our community, marketing and educating on the services that we provide.
Tammy Soodsma is the Activities Coordinator and has been working here for almost 25 years. She started out as a CNA and then about 3 years ago starting work in Activities. Tammy enjoys spending time with the residents and finding out about their life stories, as they have many years of wisdom to share. She grew up in Randolph, WI and still lives there today. Tammy enjoys spending time with her family and her dog Shilo. She hopes that her love for what she does shows to the residents and their families.
Melinda Bradley runs Central Supply and designs the nursing staff schedule. She started at this facility as a CNA in July of 2002. Since then Mindy has become a Med Tech, and now she is the Nursing Scheduler/Central Supply. It’s amazing all the friendships and wonderful memories she has made here in the last 19 years. Mindy is am a mom to a beautiful 19 year old daughter Emile and their little dog Piper. In her spare time she loves to read, spend time with her loved ones and be outdoors.
Jessica Jacobson has served the Beaver Dam Health Care Center as our MDS Coordinator for the last 2 years. Jessica obtained her Bachelor’s of Science in Nursing from Marian University in 2004, where she also majored in Spanish. Jessica is currently furthering her education by working to obtain her RAC-TC. Jessica has 8 years of acute care experience as well as 9 years of long term care experience, this experience is invaluable to the residents and staff. Jessica also enjoys spending time with her family, camping and giving of herself to the local Kiwanis as a member of the board.
Denise Beal serves the Beaver Dam Health Care Center as the Certified Dietary Manager. Denise has been in the food service industry for 34 years, in various roles. She has been with our team for the last 7 years. Denise also serves the local community center as the manager. In her free time Denise enjoys spending time with her grandkids, reading and camping. Denise further adds to our team as a member of the Beaver Dam Rotary. Denise is a vital part to the Bedrock team serving our residents and contributing to our team atmosphere.

Nancy Percifield contributes to the Beaver Dam Health Care Center as the Director of Social Services. Nancy has been in this role since November 2007, giving to our residents and staff her experience of over 25 years. Nancy obtained a Bachelor’s degree in social work from UW-Oshkosh as well as a Master’s degree in Accounting. Nancy’s role here involves completing assessments with residents, conducting care plan meetings, interacting with both the residents and their families as well as serving the short term rehab individuals with their discharge and home care needs. Nancy is an invaluable team member, assisting others with tasks and taking time out of her busy schedule to meet the needs of our residents. Nancy further contributes to our community and team by taking time out to serve as a member of the Beaver Dam Rotary.

Kathy Niles has been the Business Office Manager at Beaver Dam Health Care Center since November 2019. Kathy is in charge of keeping track of the day to day tasks relating to the financial aspect of the facility. Keeping up with accounts, insurance and the like. She enjoys making sure everything is in place for the residents stay at our facility, and making sure that part of the residents stay there is the furthest from their mind.
Tosha Quintana is in charge of Medical Records and has worked at Beaver Dam HCC for 18 years, starting out as a CNA. Tosha loves her job and her co-workers are her second family. Tosha has a husband and two children Xavier (15) and Lea (11). Her hobbies include spending time with family, reading books, and spoiling her sweet dogs, Bucky and Lulu.

Jennifer Fellin is the Director of the Alzheimer’s Care Unit and has been in long term care, specializing in dementia care, for 25+ years. She attended UW Oshkosh majoring in Special Education and is a Certified Dementia Practitioner. Jen has been at Beaver Dam HCC for 11 years and has been the key to success in dementia friendly care. She is also a Certified Alzheimer’s Support Group Facilitator through the Alzheimer’s Association, President of the Dementia Concerns Coalition of Dodge County, and a Member of Dementia Friendly Dodge.
Beaver Dam HCC is proud to have the wonderful and muti- talented Greg “Mac” MacDonald as the Director of Maintenance. Greg has an extensive background in maintenance as well as previously working in a couple facilities and nursing homes as Environmental Services Director, Maintenance Supervisor as well as purchasing and safety coordinator. He has also worked for a national motorcycle enthusiast magazine publishing articles as well as a contributing photographer, covering events throughout the midwest. Greg has also ran a restaurant, bar and event venue for several years. He is happy to be back in a nursing home setting, as he enjoys his wonderful coworkers and always looks forward to a great relationship with the facilities residents.
